The Transitions college program focuses on providing evidence-based academic support for each student to achieve college and personal success. Transitions offers supports to help navigate a college’s services and course offerings; develop social and academic skills to manage course work; communicate with students, faculty and community members; and learn other related skills necessary for each student to accomplish their goals.

Students can elect to enroll at Transitions while simultaneously attending any one of the regional colleges near our Fulton, Albany and Schoharie locations. Our partnerships with local colleges and universities helps students maintain a robust academic life where they are properly supported throughout their college journey. At each of the colleges, students can attain microcredentials, 2-4 year degrees or work through an individualized program. All students in the Transitions program enroll in a “career exploration and success” course each semester to help ensure each student is on the right path.
How to Apply to Transitions
Students and parents looking for a supportive environment to help students learn independent living skills and prepare them for college life may want to learn more about applying to Transitions. The application process starts with reaching out to us. Prospective applicants can request information for themselves or their child and start the application process. If there is a good match, we will schedule an interview with an advisor and arrange a campus tour before applicants decide to submit a formal application.

Transitions Will:
